Short term support – reablement
Reablement is a type of care at home service that provides support to individuals for up to six weeks designed to maximise an individual’s independence.
Reablement is available to individuals over 18 who:
Have been discharged from hospital back into the community
Those who may have suffered a fall or injury
Individuals who may require assistance and support re-learning household and personal care tasks
We will work with you to develop a personal support plan to help you with daily living skills and provide you with practical assistance to do this.
Reablement is provided free of charge however, if support is required after six weeks, you may have to pay a contribution. If a contribution is needed, this will be determined by a financial assessment.
