Care at home services
We will support an individual’s choice and assist them to stay at home or in a homely setting for as long as possible.
We provide a care are home service that is available 24 hours a day, 365 days a year.
The care at home service can help with daily tasks such as:
Getting washed and dressed
Preparing meals and eating
Going to the toilet
Remembering to take or taking medication
Getting into and out of bed
Care at home services may be provided by the Council’s care at home team, or it may be provided by one of our care at home partners. However, please be assured that you will receive the same standard of care regardless of your provider.
If you think you need a care at home service and are over 16, please contact Social Care Direct and ask us to look at your needs. Social care will have a conversation with you about what you think your needs are and how these could be met.
If as part of our conversation, we determine you need support from us and meet our eligibility criteria, we will work with you, to develop a personal care plan which will set out what help and support you need and how we will support your needs.
If you don’t meet our eligibility criteria, we will signpost you to other organisations that can provide you with advice, support and services either privately or voluntarily.
