Support in the community

We can support you to stay active and remain involved in your community. We can help you:

  • Get back into work

  • Join local activities

  • Make new friends

  • Access specialist services

We will support you to plan what you want to do and to achieve those outcomes, however our involvement would be time limited to help you to regain  your independence.

If we think your needs are critical or substantial, we may offer you support within our day services. If we think your needs are either low or moderate, you may be supported by the local area co-ordination team.

If you are over 16, and have a mild or moderate physical or learning disability or acquired brain injury, we can help you to:

  • Make transport on your own

  • Make plans

  • Meet friends and get to know other people

  • Look into education opportunities that may help you get work

  • Use leisure services
