Reablement

Reablement is a type of short term care at home service for adults. Reablement is for people who:

  • have been discharged from hospital back into the community
  • have suffered a fall or injury
  • need assistance and support re-learning household and personal care tasks.

We will work with you to develop a personal support plan to help you with daily living skills and provide you with practical help to do this.

Around the sixth week, the reablement co-ordinator will arrange a review visit with you and your family to discuss next steps. If you need care for longer than six weeks, we will talk to you about the different options for care.

Our team will remain involved until we find a new support service if that is what you need. We will organise a handover to make sure that all important information about your care and support needs is discussed with you and the new team.

In Scotland, any adult who has been assessed as having personal care needs will get support free of charge.
