Reablement

Reablement is a type of short term care at home service for adults. Reablement is for people who:

  • have been discharged from hospital back into the community
  • have suffered a fall or injury
  • need assistance and support re-learning household and personal care tasks.

We will work with you to develop a personal support plan to help you with daily living skills and provide you with practical help to do this.

Reablement is free of charge for the first six weeks. If you need support after six weeks, we will have a conversation with you about what your options are. This will include a conversation about money and whether you need to make a financial contribution to your care.
