Connect Here

Connect Here is a community information database. In it, you will find local and national resources that can help you manage your long term condition.

Connect here has a range of information about each group or organisation including:

  • a short summary about each service
  • contact details such addresses, email address and websites
  • where to find them on social media

Some services may need you to be referred by your GP, health and social care worker or link worker. If this is the case, the details will be with the information about the service.

Where has the Connect Here Directory gone?

The full community directory will no longer be provided in PDF form. To improve the user experience (access/navigation etc) we are currently working on getting the full database published online. We hope to have this work completed during 2025.
