Eligibility criteria

Need advice or support

If you feel you need some help and support with your health and social care needs, please contact us and we will have a conversation with you about your needs, what family and community support you have and whether you have a carer or not.

We will apply some principles to determine whether you are at risk of losing your independence if your needs are not met.

The principles are:

  • Critical

    Risk of major harm / danger to a person or risk to independence and this is likely to occur now or in the next few days

  • Substantial

    Risk of significant impairment to the health and well-being, or significant risk to independence and this is likely to occur now or in the next three months

  • Moderate

    Risk of some impairment to the health and well-being of a person, or some risk to independence and this is likely to occur now or in the next 12 months

  • Low

    Low risk to independence / health and well-being

If as part of our conversation with you, we feel that your safety and loss of independence are deemed as high risk, we will discuss what support is needed. If the risk is deemed lower, we will provide you with information, advice and signpost you where necessary.
